How to find a provider for eBenefits Summit and Essential Plans

If you are enrolled in the eBenefits Summit product:
  1. Visit:  https://www.multiplan.com/webcenter/portal/ProviderSearch
  2. Click on "Select Network"
  3. Select "PHCS"
  4. Select "Practitioner and Ancillary"
  5. Check "Remember my network"
  6. You can now use the search box to perform various searches by name, specialty, practice, etc.
Please note:  Preventive services performed at hospitals will be declined for coverage.

  1. Visit:  https://www.multiplan.com/webcenter/portal/ProviderSearch
  2. Click on "Select Network"
  3. Select "PHCS"
  4. Select "Preventive Services  Only"
  5. Check "Remember my network"
  6. You can now use the search box to perform various searches by name, specialty, practice, etc.
