Are 1095 forms issued for the SALA Healthcare Program
No, they are not. The 1095 forms are usually issued to document participation in a
market place plan or an employer sponsored plan. Most members will not need 1095, but in the case that your state does require some proof of participation, SALA can provide a payment
history report and cover letter describing the program for you. Requests should be directed to the help center by emailing hpsupport@smallassociations.org.
